copies required at registration. We
recommend you print an additional copy for your
own records.

online from a computer that has an attached printer.
Print the forms
-- two singer registrations will print. Both are
required at registration.
Sign all forms and be
sure to have your singer review and sign the code
of conduct.
BRING
ALL PRINTED AND SIGNED COPIES WITH YOU TO REGISTRATION.
You should have two copies of the singer registration
information sheet and one copy of all other registration
forms.
